Job Description:
Coordinate and oversee the delivery of high-quality nursing care to patients in their homes, ensuring adherence to established care plans and protocols.
Supervise and support a team of home healthcare nurses, providing guidance, mentorship, and feedback to enhance clinical performance.
Conduct patient assessments as needed, ensuring individualized care plans are appropriate and updated based on patient conditions and needs.
Collaborate with physicians, therapists, social workers, and other healthcare professionals to ensure comprehensive and holistic care.
Ensure compliance with organizational policies, home healthcare regulations, and accreditation standards.
Monitor patient outcomes and satisfaction, addressing any concerns or complications promptly.
Assist with staff scheduling, workload assignments, and resource allocation to meet patient needs efficiently.
Act as a liaison between patients, families, and the healthcare team to facilitate effective communication and care coordination.
Provide education and support to patients and families regarding disease management, medication use, and preventive care practices.
Oversee the documentation of patient care activities, ensuring accuracy, completeness, and confidentiality in medical records.
Ensure staff adherence to infection control measures and safety protocols during home visits.
Lead quality improvement initiatives and participate in clinical audits to enhance care delivery and operational efficiency.
Address and resolve staff or patient concerns, fostering a positive and supportive environment for both employees and clients.
Participate in the recruitment, onboarding, and training of new home healthcare staff, ensuring competency and readiness for fieldwork.
Monitor and maintain necessary equipment, supplies, and medications for home care services.
Evaluate staff performance regularly, providing constructive feedback and identifying opportunities for professional development.
Respond to emergencies or critical situations during home visits, providing guidance or direct care as needed.
Develop, implement, and review policies, procedures, and care standards specific to home healthcare services.
Advocate for patient rights, promoting dignity, respect, and patient-centered care in all interactions.
Participate in departmental meetings, case conferences, and interdisciplinary team discussions to ensure optimal care planning and execution.
Report regularly to senior leadership on patient outcomes, staff performance, and operational challenges within the home healthcare program.
Promote a culture of accountability, compassion, and continuous improvement among the home healthcare team.
Education:
Bachelor’s Degree in Nursing
Valid registration/ license as a Nurse
Must have Nursing Specialist or Higher registration in the Saudi Council or Higher
Experience:
At least ONE (1) recent years clinical nursing of unit-in charge experience.
More than TWO (2) years clinical nursing experience as a Registered Nurse in the required unit/department.
